Night Driving After Refractive Lens Exchange: What to Expect

Night driving is one of the most common concerns patients raise before refractive lens exchange. How will my vision hold up in the dark? Will oncoming headlights bother me? The answer depends largely on which lens you choose. Watch our overview below, then read on for a clear breakdown of what to expect.

How Modern Lenses Compare to Your Natural Lens

Starting around age 50 to 60, the natural lens begins filtering out light, gradually reducing contrast and clarity, including at night. A modern intraocular lens restores that light transmission significantly. EDOF lenses perform comparably to a healthy lens in a 15 to 20 year old, and the Light Adjustable Lens transmits light similarly to a 5 year old’s natural lens. For most patients, night vision after RLE is better than it was before surgery.

Multifocal Lenses and Night Vision

Multifocal lenses work by splitting incoming light to correct distance, intermediate, and near vision simultaneously. The benefit is meaningful: most patients with multifocal lenses do not need reading glasses at all.

The trade-off involves night driving. A small number of patients, roughly 1 to 2%, find that distance vision quality in low light or at night is not quite what they expected. This is the most common reason a multifocal lens is removed. When that happens, we typically wait about three months to confirm the symptoms are persistent rather than part of normal adaptation. If they are, the lens can be removed and replaced with a non-ring design lens.

EDOF Lenses and Night Vision

Extended depth of focus, or EDOF, lenses use a different optical design that does not split light. Because of that, distance and intermediate vision quality, including at night, is consistently strong. Most patients find their low-light and night driving vision is noticeably improved compared to before surgery. Most patients will still use reading glasses for fine near tasks, but the overall clarity, especially in the dark, is one of the strengths of this lens design.

The right lens depends on your lifestyle and what matters most to you. If night driving is a priority, that is an important part of the conversation during your consultation. Our team works through those details with you so you go into surgery with a clear picture of what to expect.
